About the macros: the two Add buttons only copy a row between sheets inside the workbook. They never touch other files, the internet or your settings. Click Enable Content when Excel asks; the Macros sheet walks you through the one-time Unblock step for downloaded files. Prefer no macros? Each form has a ready row you can copy and paste instead.

Works in desktop Microsoft Excel (Microsoft 365 or Excel 2016 and later, Windows or Mac). The Add buttons need desktop Excel, as macros don't run in Excel for the web. Instant digital download, nothing is shipped.
